Unsubscribe is not working

You used the same code and form as in the documentaton but when we click on the 'unsubscribe button' we get 'Form not found'. Can you give us an idea what may be happening since we have the button in place and using the same code in the user manual but the form not found as us not clear on an answer. Please advise
