Help with Actions and Forms

I want to have a button in the email where someone can request a call, this will then be routed to the sales rep for them to deal with directly. I have created a form and an action and tried both with a link action and through the form. I’m wanting to use the service email action. But what I am wanting to have in the email that is sent to the sale rep is some info from the data base IE the name of the person. I have tried using the [agn] tags like [agnFIRSTNAME], [agnEMAIL] and [agnDB column=â€
