Problem: Double Opt-In

I have a strange issue which I am unable to identify. I have set up the forms and actions for double opt-it by just modifying the English samples provided in the original OpenEMM install. Everything works fine and I received my confirmation email correctly. However, when I click the link to confirm, I receive the error portion of the form. When I look in the database, the user is still "Waiting for Confirmation".
In the final form, my entry action is doi_user_confirm and there is no action leaving the form. On the setup of the doi_user_confirm action, the only step is Double Opt-In Confirmation. The usage is setup for Forms and Links.
Do you have any advice? Are there any log files I can be checking for why it won't work. I am using 5.5.1 release of OpenEMM.
